Physical exams can be performed by any medical professional who provides them as part of their regular practice.
The Family Medical Clinic will again provide sports physical exams for $25 ($35 for exams done at the Clinic). Please have your medical history portion of the exam form completed and signed before attending the exam. The fee is payable by either cash or checks made payable to Family Medical Clinic. Some insurances accepted. Please check with the Family Medical Clinic for acceptance.
Students can turn their completed Activity Forms in at the clinic to Mr. Holland.
