Greetings!  We would like to welcome you to the Activities Program of the Townsend School District.  As with all programs, there is a certain amount of paperwork that must be completed before an individual can participate. 

For all students entering Grades 7-12 for the academic year 2017-2018, there will soon be a new link to the 2017-2018 Participant Activity Booklet (pdf) which includes ALL the forms and handouts that need to be read and/or completed BEFORE a student will be allowed to begin practice or participation.  The booklet can also be picked up from the Activities Office located in the High School during regular business hours.

This booklet must be completed by those wishing to participate in the following activities:  HIGH SCHOOL - Basketball (Boys and Girls), Cheerleading, Cross Country (Boys and Girls), Football, Golf (Boys and Girls), School Plays (No physical exam required, but medical history must be completed), Speech and Drama (No physical exam required, but medical history must be completed), Tennis (Girls only), Track (Boys and Girls), Volleyball (Girls only), and Wrestling;  MIDDLE SCHOOL - Basketball (Girls and Boys), Cross Country (Girls and Boys), Football, Track (Girls and Boys), and Volleyball (Girls only).

Activity Physicals for those wishing to participate in sports must be completed BEFORE a student can participate in an activity.  The approved MHSA form must be used.  This can  be found in the Student Participation Booklet.  A physical exam form completed after 1 May 2017 is valid for the 2017-2018 school year.
All other forms must be completed by the student and parent/guardian.  The completed packet needs to be turned in the Activities Director BEFORE the first day of practice. This includes all necessary signatures and initials.

Townsend Schools require that any student participating in an athletic program, including cheerleading, must show proof of health insurance before participating.  If you do not have health insurance you can purchase appropriate coverage from the company below.  The school district is not responsible for medical coverage of injuries that occur during participation in school sponsored activities.
